Medicaid Fee-for-Service (FFS) provides home health care through dedicated waiver programs that cover 24-hour long-term care for eligible individuals.

● The Nursing Home Transition and Diversion (NHTD) Medicaid Waiver provides support and services to those who are either being transitioned from a nursing home or being cared for at home in lieu of a nursing home.
